Session Survey Setup: How to create a Survey or Quiz

1. To make edits to the event platform page, log into the event platform and access the admin page

EVA Tip: You can only make changes to the event platform with Admin access


2. On the left column, click on Sessions, and in the drop-down menu that appears, click Session Survey Setup



3. Click the Add Form button



4. Select the Quiz Type from the dropdown menu


5. Enter the name of the form



6. From the drop-down menu, select the Survey Fields

7. Select the Status from the drop-down menu


8. To save your changes and add the Survey, click the Add button. To discard your changes, click the Close button.
